The New / First Time Application process is for applicants :

Who are applying for a passport for the first time, OR

Whose prior passport was issued at age 15 or under, OR

Whose prior passport was issued more than 15years ago, OR

Whose expired passport is not in their possession.

Applicants who lost a passport that is still valid, must follow lost or stolen instructions, Click Here

One of the following:

Old Passport, OR

Official government issued Birth Certificate (if born in the US), OR

Certificate of Citizenship (if born abroad to US parent (s) ), OR

Naturalization papers (for US citizens through naturalization)

Proof of citizenship must be an original or certified copy. No photocopies or notarized copies are accepted.

The original document will be returned to you with your completed passport.

One of the following :

Previously issued, undamaged US passport

Valid Driver's License, issued more than 6 months ago

Current Government ID  (city, state or federal)

Current Military ID

If the applicant does not possess a valid driver's license (state ID / Military ID), or if license was issued LESS than 6 months ago (even renewal). additional proof of identity are required.

Please note that applicants age 16 or 17 without a valid driver's license can have a parent co-sign the application. in this case a Birth Certificate is required.                                                                                                                                                                                                                                                                                      

This Letter of Authorization gives Visa Obtainers permission to represent you at the US Department of State 

Each Letter of Authorization MUST be handwritten.

Original signature is required.

All fields are required to be filled in the Letter of Authorization.

Check the first two boxes, DO NOT check the third box , this will cause delay.

In the courier company name field, please handwrite Visa Obtainers Inc.

One passport type photograph is required. Photo must be recent, taken within the last six months, 2x2 inches, front view, full face, in color and on plain light/white background

Include one of the following:

Copy of travel itinerary, from airline or travel agency with applicant name, travel date and flight info., OR

Copy of ticket showing foreign departure

If travel is for business and neither of the above is available, a letter from employer stating you have immediate international travel.

Proof of departure is not required for regular service.

The last page of the application is a print summery page, please scroll to the bottom of the left corner of the page and click create form, you will be then able to print your application.

Print TWO copies, single sided only.

DO NOT sign your DS.11 application until you are instructed to do so by your local Passport Acceptance Agent. (explained at the passport execution process step) 

A correctly formatted DS.11 application form should have a BAR CODE on the top left corner.

There is additional $ 30 government fee for the passport card. 

The passport card cannot be expedited at this time and will be mailed to you directly by first class mail from the US Passport Agency

Get Application Authorization (passport execution process)

(Applicant Identity Verified  and DS.11 Signature Witnessed)

Once you have collected all the documents required to apply for a new / first time passport you MUST now have your application authorized by local Government official called Passport Acceptance Agents located in Post Offices and Courthouses.

The Passport Acceptance Agent job is:

Witness the signing of the DS.11 passport application form, AND

Verify the applicant's proof of citizenship, AND

Verify the applicant's ID, AND

Verify photo is for the applicant, AND

Administer the passport oath, AND

Collect Government fees, AND

Seal all documents into a sealed envelope.

US Passport Agency Fees  

While at the local Post Office or Court House the following Government fees will be collected:

$ 170 Passport Fee, a check or money order to US Department of State.

$ 35 execution fee , a check or money order to your local Post Office or Court House.

Please write your name and date of birth on the check or money order.

LIST OF DOCUMENTS:

please make photocopies of your DS.11 passport application form, your Letter of Authorization and your proof of departure.

The following items MUST be placed inside the sealed envelope executed by the Acceptance Passport Agent at the Post Office or county court house, then all documents MUST be placed inside a FedEx envelope for shipping to Visa Obtainers, Inc.

Proof of Citizenship, AND

The application form DS.11, AND

The passport type photograph, AND

The Government Fees, AND

The first copy of Letter of Authorization, AND

The first copy Proof of Departure, AND

Any other required document.

The following items are NOT sealed inside the sealed envelope and MUS be placed inside your FedEx envelope for shipping to Visa Obtainers, Inc.

The second copy of the DS.11 passport application form (unsigned), AND

The second copy of the Letter of Authorization, AND

The photocopy of the proof of departure, AND

Copy of your online order form.

Do not send your documents to Visa Obtainers until you have your documents sealed by your local Passport Acceptance Agent.
