A copy of the information pages of your passport.
Your passport must be signed and valid for at least 6 months.
The Embassy can ask for your original passport.
Two passport type photographs is require. Photo must be recent, taken within the last six months, full face, in color and on plain / white background.

A copy of your itinerary from airline or travel agency . The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information.
Copy of recent bank statement is required.
A copy of your Permanent Resident Card (Green Card) or valid US visa and I-94 is required.
If you entered the USA after April 26th 2013 you should print the electronic I-94 form.

Proof of medical insurance is required.
Letter from employer stating the applicant is leaving for vacation.
If visiting a family member or a friend, invitation is required.
